It sounds like your whirl and pinion are worn out. They are the two gears that you see after removing the horn's cap. The whirl lassos the needle, while the pinion turns the whirl.

Hand turning it on the hand wheel will not exert the torque as when engaging the motor. Therefore, the worn out gears are likely to slip out of synchronization when motorized.
